IMPORTANT COVID-19 INFORMATION: Dr Kaplan and his staff are working hard to follow CDC & ADA's recommendations to provide you with the safest environment for your oral surgery needs. A few things to keep in mind - a mask will be required to enter our office, only the person receiving treatment will be allowed into the office unless the patient is a minor or needs a caregiver with them. Please call the office at 401-861-5100 before entering the building for your appointment, so we can manage social distancing of our patients! We appreciate your patience as we will be asking health questions, sometimes repeatedly, to screen each and every patient.

 

Scheduling your office visit.

When calling for an appointment with our office, we ask that you schedule a consultation before most surgeries. We will ask you to provide personal information along with your health history. Copies of both your dental and medical insurance cards will be taken. At this consultation appointment, we will discuss the type of procedure(s) you will be receiving along with your options of anesthesia. Minor procedures under local anesthesia can be performed on the same day as the consultation. However, complex procedures that require a treatment plan will usually need to schedule an additional appointment for surgery.

We will need the following items at your consultation appointment:

  • Referral slip from your general dentist or doctor
  • Panorex (if available)
  • Medication List
  • Dental and Medical Insurance Cards

If the patient is under the age of 18 or is not financially responsible, a parent/guardian must accompany the patient to all appointments.
